At Pediatric Ear, Nose & Throat of Atlanta our eight physicians are board certified and have completed an additional fellowship in pediatric otolaryngology. Together, we have more than 100 years of combined experience in managing children with both common and complex ear, nose and throat conditions which require the expertise, compassion and care of a pediatric ENT physician.

Preparing for Your First Office Visit

If you are preparing for your first visit to Pediatric Ear, Nose and Throat of Atlanta, please review the following information so that you are prepared. This information and the provided forms are available for you to complete prior to your visit. By your completing these forms prior to your visit, we will be able to see your child more quickly and streamline the appointment process.

Because our physicians specialize in the treatment of complex pediatric conditions, you may experience a longer wait time during your first visit. We strive to provide comprehensive service to all of our patients and want to ensure that your visit with us is thorough and complete, so please plan accordingly.

Convenient parking is available at all of our locations. There is a small parking fee at the main office which is located on the Children’s Healthcare of Atlanta at Scottish Rite campus.

The Parent Checklist

When you make your appointment, the scheduler will direct you to our Website so that you can download the necessary forms online, complete and bring to your child’s first visit.

  • A parent must accompany a child to the first visit. Grandparents and other designated individuals may accompany a child to any appointment after the initial visit.
  • Arrive 20 minutes prior to your scheduled appointment time.
  • If you need to cancel or reschedule, try to provide at least 24 hours’ notice.
  • Bring your insurance card and your identification with you.
  • Please check to make sure your insurance covers your visit.
  • Be prepared to pay any co-payment, co-insurance or payment toward your remaining  deductible, at the time of your visit.
  • Refer to your insurance handbook for specific benefits (what your insurance covers and pays) and limitations (what your insurance does not cover or pay).
  • Bring any medical records that you have available.

Insurance

We participate with most HMO and PPO insurance companies. Please refer to this list to ensure that the physician you are seeing is covered by your particular insurance. As a convenience to you, we will verify your insurance using the information you provide when you scheduled your child’s first appointment.

If we are unable to verify your insurance, you will be responsible for payment in full at the time of the visit. Co-payments, co-insurance, and deductibles are also due at the time of visit. Also, please refer to your insurance handbook for specific benefits (what your insurance covers and pays) and limitations (what your insurance does not cover or pay) of your health insurance policy.

My PENTA Page

For your convenience, PENTA also has a patient portal where you can access all of your information, update your contact information, receive secure and private communications, and pay your bill online. Once you have registered, you can set up your personal account on My PENTA Page. Once registered, you can access your information any time, day or night.
